USA HOCKEY PATCHES AND AWARDS (FOR TAHA AFFILIATE ASSOCIATION MEMBER PLAYERS & TEAMS ONLY)
As you may know, USA Hockey provides players with certain achievement awards. The three types of awards are noted below (Only one per patch, per player, per season):
- Hat Trick - Patch for any player who scores 3 goals in a single USA Hockey sanctioned game
- Playmaker - Patch for any player who records 3 assists in a single USA Hockey sanctioned game
- Zero Club - Patch for any goalie who records a shutout in a single USA Hockey sanctioned game
**ONLY ONE FORM SUBMISSION PER PATCH, PER PLAYER, PER SEASON IS PERMITTED**
SUBMISSIONS ARE ACCEPTED ONLY FROM TAHA AFFILIATE MEMBERS AND WILL BE DISTRIBUTED AFTER THE END OF EACH SEASON ONCE VERIFIED.
In order to receive a patch, a TAHA Member Association or TAHA Member Team Official is required to send email to awards@tahahockey.org at the end of the season that contains the following information:
- Player Name
- USA Hockey Registration #
- Player Category (e.g. youth, girls, etc.)
- Award Requested
- Game Date & Time
NOTE: If request is for multiple patches, a spreadsheet may be included that contains the details above for each player/patch. However, individual game sheets for each record in the spreadsheet must be included.
Player awards will be distributed under the following conditions:
(1) The email and spreadsheet (if attached) includes the required information above for each player.
(2) The recipient is a registered player.
(3) The game was in league competition (not exhibition) or a sanctioned tournament involving registered USA Hockey teams.
(4) The game was officiated by a USA Hockey referee.
(5) Eligible player categories: Over-30, adult non-checking, juniors, girls and youth.
(6) A copy of the scoresheet is supplied.
(7) Limit of one (1) of each award per player, per season.
As a reminder, please DO NOT submit multiple online requests for the same player/game/award. Please DO NOT submit a request if you ARE NOT a TAHA Member Association or TAHA Member Team Official.
